Before You Submit

Verify the information you have provided is correct, including the student’s name and grade level.

Indicate the reason for the appointment (academic, personal, social, scheduling, college/career, etc.).
✔
If you are a parent/guardian include your phone number and email address for follow-up.
What Happens Next
The Guidance Counselor will review your request and follow up within 1–2 school days. Appointments will be scheduled at a time that works best for the student and minimizes class disruption whenever possible.
